We're currently working on module for our internal team to use to get documented approvals for designs and certain documents from our clients The module works like so:
1. Document is uploaded
2. An email with a link is sent to the client
3. Client visits link and is authenticated with a special one use token (or client username and password)
4. Client views file and clicks on approve or disapprove and fills in initials
-Approval requires agreeing to terms which you will be able to edit
-Disprove - will load a comment box for reason for disapproval
5. The approval is stored in an database with the date, file name, and name initial of client
6. An email confirmation is sent to the file uploader and client with document approval details
We're considering making it available to other activecollab users by selling it from our website. But before we take the time to make it commercially available we want to gauge the interest of you, the users.
Also if you have any suggestions or comments, please feel to post them.

We use currently aC to track Magazine ads (Tracking approvals, plus using it as a checklist, to make sure not any ads are missed when we go to publication). One item that would be benefit us is the ability to have a list of approvals in one project & be able to tell at a glance who has approved & who has not.
That is the only area that I can see where we would use it different than you have mentioned above. Basically what you have outlined would be perfect.

I think when someone start to test something, then he can post some request and suggestions
