Subdomains and subfolders count as URL-s. google.com, labs.google.com, google.com/labs and labs.google.com/docs - they are all different.

One setup on your own machine for development and testing is allowed.

You can request URL change on your profile page. When change is approved you will need to remove old setup from the old URL within 48 hours.

Yes, you do. License is per URL, not per domain (different subdomains and subfolders count as different URL-s).
